Dog wobbly or off balance: what it means and when to call the vet

Emergency — go now

A dog that suddenly stumbles, tilts the head, walks in circles or looks drunk needs a vet the same day. Causes range from inner-ear problems to poisoning and spinal or brain disease.

Common causes

  • Idiopathic vestibular disease ("old dog vestibular syndrome")
  • Middle or inner ear infection
  • Poisoning, especially cannabis edibles
  • Intervertebral disc disease
  • Low blood sugar
  • Stroke or brain tumours
  • Side effects of some medicines

What you can do at home

  • Keep your dog in a safe, padded area away from stairs
  • Carry or support with a towel sling
  • Tell your vet about anything your dog could have eaten
  • Hand-feed and offer water if nauseous

What the vet may do

  1. Neurological examination
  2. Ear examination
  3. Blood tests including sugar
  4. Treat poisoning and nausea
  5. MRI if brain or spinal disease is suspected

Prevention

  • Store cannabis products, medicines and alcohol securely
  • Treat ear infections promptly
  • Keep long-backed breeds lean and limit jumping

The details

Older dogs who suddenly develop a severe head tilt, rolling eyes and a drunken walk often have idiopathic vestibular disease. It looks like a stroke and is frightening, but most dogs improve markedly within days and recover over a few weeks with supportive care.

That said, many other causes look similar, including poisoning and serious brain or spinal disease. Cannabis poisoning is now one of the most common causes of sudden wobbliness in dogs, often with dribbling urine and sensitivity to sound. Tell your vet honestly about possible exposure — they are there to help your dog, not to judge.
